Registration 

1.  Event Registration

 

To register for the event please complete the following steps:

 

1. Fill out the registration form
2. Save a copy for your records
3. Attach registration form and email to Addiction.Prevention@albertahealthservices.ca 

 

 

 

2. Hotel Reservation

 

If you are a community partner we will be booking a hotel room for you on your behalf, if you indicate on your travel pre-approval that you require one. We will send you a confirmation number closer to the date of the event. 

 

Alberta Health Services Staff, if you require a hotel room, please book directly through the Clarion Hotel at 403-291- 4666. Indicate that you are with Alberta Health Services- Coalitions Connect for a group rate.
